Notice of Privacy Practices
Effective Date: August 1st, 2026
Your Information. Your Rights. Our Responsibilities.
This Notice of Privacy Practices describes how Trouvaille Marin (“Trouvaille Marin,” “we,” “our,” or “us”) may use and disclose your protected health information (“PHI”), and how you can access and manage your health information. Please review it carefully.
We are committed to protecting the privacy of your personal health information and maintaining your trust. As a concierge primary care practice, we believe that a strong physician-patient relationship is built on respect, confidentiality, and thoughtful stewardship of your information.
Your Health Information
Your health information includes information about your health, medical conditions, treatments, medications, healthcare services, and payment for healthcare services. We create and maintain records documenting the care and services we provide to you. These records allow us to provide high-quality medical care, coordinate your healthcare, and comply with legal requirements.
How We May Use and Disclose Your Health Information
We may use or disclose your health information for the following purposes:
Treatment
We may use and share your health information with healthcare professionals involved in your care to provide, coordinate, and manage your medical treatment. This may include consultation with specialists, coordination with other healthcare providers, and communication related to your care.
Payment
We may use and disclose health information as needed to process payments, manage your account, verify eligibility for services, or collect payment for healthcare services. Because Trouvaille Marin operates as a concierge primary care practice, some services may be provided through a membership model and may not be billed through health insurance.
Healthcare Operations
We may use and disclose your information for practice operations, including quality improvement, care coordination, administrative activities, compliance reviews, and evaluating the effectiveness of our services.
Appointment Reminders and Communications
We may contact you using the contact information you provide to communicate about appointments, care coordination, test results, health-related services, and other practice-related matters. This may include phone calls, voicemail messages, text messages, email, and secure patient portal communications.
Treatment Options and Health-Related Services
We may communicate with you about treatment options, preventive care recommendations, wellness services, or other health-related services that may be of interest to you.
Individuals Involved in Your Care
With your permission, we may share relevant information with family members, caregivers, or other individuals involved in your healthcare or payment for your care.

We may disclose your health information when required by federal, state, or local law, including:
-
Reporting certain public health concerns
-
Responding to legal proceedings or law enforcement requests as permitted by law
-
Reporting suspected abuse, neglect, or domestic violence when required or permitted
-

Other Uses and Disclosures Requiring Your Authorization
Uses and disclosures of your health information not described in this Notice or otherwise permitted by law will only be made with your written authorization.
You may revoke your authorization at any time in writing, except to the extent we have already relied upon it.
Certain uses and disclosures, including most uses of psychotherapy notes, certain marketing communications, and disclosures involving the sale of health information, require your authorization under applicable law.
